Job Details

1. Evaluates health, functional and psychosocial status of Expressions residents. Participates in the development of individualized service plans for residents when they move into our community. Proactively manages care and services for each resident, maximizing the residents opportunity to remain in the assisted living environment. 2. Provides day to day supervision to Expressions (memory care) team members including hiring, training, scheduling and performance management. 3. Creates and leads monthly programming for the Expressions neighborhood utilizing the Expressions focus plan and product materials. 4. Ensures the daily delivery of life enrichment activities in the Expressions neighborhood incorporating the 5 Signature areas of programming. 5. Provides initial and ongoing training of new team members to Expressions neighborhood using the Passport Training program as well as completing any required state specific training(s).
