Description: We are seeking a dedicated Home Health Physical Therapist. This position requires providing care to patients in their homes. You will have a 1:1 patient ratio and may visit up to 6 patients per day. Productivity points will be standardized at 30 points. The role does not require weekend work or on-call duties. The facility is located within a 50-75 mile radius and accommodates local applicants. A primary series COVID vaccination plus one booster is required. You will be responsible for patient evaluations and developing tailored rehabilitation plans. Candidates must possess a strong understanding of home health practices and protocols. This position does not allow for shifts of 4x10s or 3x12s. All travelers should be prepared to meet productivity expectations based on patient care needs. Evaluate patients with a variety of conditions, including post-operative rehabilitation and chronic illnesses. Demonstrate effective communication with patients and families.
