• Shift: 8-hour day shifts (08:00 - 16:30), Monday through Friday

• Weekly Estimated Pay: $2,170 – $2,330 (competitive travel OT pay)
• Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)

Qualifications / Job Requirements

• Minimum 1 year of skilled Home Health Occupational Therapy experience (home health OT experience required)
• BCLS (Basic Cardiac Life Support) certification required
• Experience with OASIS documentation and EPIC charting required
• Reliable transportation required (travel within a 50+ mile radius may be necessary)
• Primary COVID-19 vaccination series and one booster required
• Ability to provide two supervisory references covering at least 1 year of employment within the last three years
• Licensed Occupational Therapist (OT) in Oregon preferred; pending state license accepted (Oregon OT license pending accepted)

Key Responsibilities

• Provide comprehensive home health occupational therapy services to adult and geriatric patients in Medford and surrounding areas
• Manage a caseload of up to 6 patient visits per day (30 productivity points standard)
• Perform patient assessments, develop individualized care plans, and implement therapeutic interventions focused on ADLs, mobility, safety, and adaptive equipment
• Document all care and progress using EPIC and OASIS charting systems to ensure accurate home health documentation
• Collaborate with nurses, physical therapists, speech therapists, social workers, and physicians to coordinate interdisciplinary care and optimize patient outcomes
• Educate patients and caregivers on therapeutic techniques, home safety modifications, and adaptive strategies
• Maintain compliance with Medicare, state regulations, and facility policies for home health services
