Home Health Occupational Therapist position in Medford, OR and surrounding areas. This is a 1:1 ratio assignment where the traveler may visit up to 6 patients per day with a 30 productivity points standard. Traveler is expected to have their own transportation, and the system does a good job grouping clients to minimize drive and windshield time, with an occasional maximum radius of 50 miles.

- Shift / Schedule: 8-hour days (facility cannot accommodate 4x10s or 3x12s)
Home Health Occupational Therapist position in Medford, OR and surrounding areas. This is a 1:1 ratio assignment where the traveler may visit up to 6 patients per day with a 30 productivity points standard. Traveler is expected to have their own transportation, and the system does a good job grouping clients to minimize drive and windshield time, with an occasional maximum radius of 50 miles.
Key Responsibilities
- Visit up to 6 patients per day
- Maintain 1:1 patient ratio
- Achieve 30 productivity points standard
- Provide skilled home health occupational therapy services
- Use Oasis charting system
- Chart using EPIC/Oasis
- Proficiency with Oasis system (required)
- EPIC charting experience
- Home health assessment and treatment planning
- Patient communication and education
- Documentation and compliance with regulatory standards
- BCLS certification required
- Minimum 1 year of skilled home health experience required
- First-timers not accepted
- Traveler must have own transportation
- Travel radius: occasional maximum of 50 miles
- Free parking available
- IRS Standard mileage reimbursement provided
- Facility requires primary series COVID vaccination in addition to one booster
- Any color scrubs acceptable
- Charting: EPIC/Oasis
- Type of Facility: Home Health
- No weekend required
- No on-call required
- Will not float between like units
- Not open to accommodating block schedule
- Locals accepted (considered "local" within 50-75 mile radius at same rate)
- Pending license accepted
- RTO Restrictions: Confirm with hiring manager
- Guaranteed hours: Facility can call off UP TO one shift per pay period (every two weeks)
