Details Home health setting serving Medford, OR and surrounding areas Day shift, 5x8-hour schedule (8:00 AM – 4:30 PM); 4x10 and 3x12 schedules are not available Caseload of up to 6 patients per day with a standard productivity expectation of 30 points 1:1 patient ratio Charting via Epic and OASIS No weekend or on-call requirements Travelers are expected to float between all facilities within the state; refusal to float may result in assignment termination Occasional patient visits may require travel up to 50 miles, though scheduling is designed to minimize drive time Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate Requirements Minimum 1 year of skilled home health PT experience required OASIS experience required Epic EMR experience required BLS certification required Active PT license required (pending license accepted) Must have reliable personal transportation Primary COVID-19 vaccination series plus one booster required At least 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration SSN and DOB required for consideration License required if applicable Additional Information First-time travelers are not eligible for this position Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility RTO is minimal; a few days may be approved, but significant RTO requests may impact eligibility Any scrub color is acceptable Parking is free Mileage reimbursement is provided at the IRS standard rate American Traveler Job ID P-694475.
