Details Home health setting serving Medford, OR and surrounding areas 8-hour day shifts, Monday–Friday; 4x10 and 3x12 schedules are not available Up to 6 patients visited per day with a standard productivity expectation of 30 points 1:1 patient ratio Charting via Epic and OASIS No on-call or weekend requirements No floating between like units within the home health setting, though travelers may be expected to float between facilities within the state Local candidates within a 50–75 mile radius are accepted at the same rate Requirements Minimum 1 year of skilled home health PT experience required OASIS experience required Epic EMR experience required Current BLS certification required Active PT license required (pending license accepted) Primary COVID-19 vaccination series plus at least one booster required Must have reliable personal transportation Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration SSN and DOB required for consideration License required if applicable Additional Information Responsibilities include evaluating and treating home health patients across Medford, OR and surrounding areas Scheduling is organized to minimize drive time, though occasional travel of 50 miles may be required Any color scrubs are acceptable Parking is free First-time travelers will not be considered for this position Former permanent staff must wait at least 3 months before returning as a traveler and must reside more than 50 miles from the submitting facility RTO is minimal; a few days may be approved but significant time-off requests are discouraged Block scheduling is not available American Traveler Job ID P-694477.
