Minimum Requirements Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program. Active Oregon Occupational Therapist license required. BLS/CPR certification required (American Heart Association or equivalent). Minimum experience providing OT services in home health or community-based settings preferred. Ability to perform OT evaluations, subsequent visits, starts of care, recertifications, resumptions of care, and discharges. Valid driver's license and reliable transportation for patient visits. Mileage reimbursed between visits as applicable. Minimum education requirement: Bachelor's degree.
