Details Home health setting serving Medford, OR and surrounding areas Standard caseload of up to 6 patients per day with 30 productivity points as the standard 1:1 patient ratio 8-hour day shifts, Monday–Friday; no weekend requirement No on-call requirement Travelers are expected to float between all facilities within the state; refusal to float may result in assignment termination Charting via Epic and OASIS Reliable personal transportation is required, as visits may occasionally extend up to 50 miles from home base Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate Requirements Minimum 1 year of skilled home health PT experience required Current BLS certification required Epic EMR experience required OASIS experience required Active PT license required; pending license accepted Primary COVID-19 vaccination series plus at least one booster required Must have reliable personal transportation License required if applicable Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration SSN and DOB required for consideration Additional Information First-time travelers will not be considered for this position The scheduling system groups clients geographically to minimize drive time and windshield time Any time off requests must be disclosed prior to or at the time of submission; requests made after submission or after an offer may be denied Former permanent staff must wait at least 3 months before returning as a traveler and must not live within 50 miles of the submitting facility Any color scrubs are acceptable Parking is free American Traveler Job ID P-694478.
