Details Home health setting serving Medford, OR and surrounding areas 5x8-hour day shifts (8:00 AM – 4:30 PM); 4x10 and 3x12 schedules are not available Caseload of up to 6 patients per day with a standard productivity expectation of 30 points 1:1 patient ratio Charting is done in Epic and OASIS No weekend or on-call requirements Travelers are expected to float between all facilities within the state Occasional patient visits may require driving up to 50 miles, though the system is designed to minimize drive time and windshield time Local candidates are accepted; candidates within a 50–75 mile radius are considered local and are eligible at the same rate Requirements Minimum 1 year of skilled home health PT experience required Current BLS certification required Epic EMR experience required OASIS experience required Active PT license required (pending license accepted); license required if applicable Must have reliable personal transportation Primary COVID-19 vaccination series plus one booster required Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting SSN and DOB required for consideration Additional Information First-time travelers will not be considered for this position Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility Any RTO requests should be disclosed upfront, as post-offer RTO requests may be denied Any color scrubs are acceptable Parking is free at facility locations Mileage reimbursement is provided at the IRS standard rate American Traveler Job ID P-714795.
