Job Title: Physical Therapist, Home Health/HospiceLocation: Grants Pass OR, 97527Contract: 3 months (potential for extensions)Shift: 8am - 4:30pm; 40 hours per weekCoverage Areas: Josephine CountyResponsibilitiesPerform Physical Therapy visits, including Start of Care (SOC), initial PT evaluations, recertifications, Resumption of Care (ROC), and required supervisory visits.Develop, implement, and modify individualized treatment plans to improve patient mobility, strength, balance, and functional independence.Accurately document visit notes, outcomes, and productivity metrics in the HomeCare Homebase (HCHB) EMR, meeting productivity expectations of 30 points per week.Educate patients and caregivers on exercise programs, safety, fall prevention, and use of assistive devices in the home environment.Coordinate care with physicians, nurses, and interdisciplinary team members to ensure continuity and quality of patient care.Productivity Expectations30 points per week.QualificationsDoctorate in Physical Therapy.Preferred skills and attributesHome Health experience.Experience with HomeCare Homebase EMR.Other DetailsWill mileage be reimbursed between visits, from first patient to last patient (Y/N)?
