Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 07/13/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Estimated Gross Weekly Pay: $ 2826.00 Opportunity: Home Health PT - Medford OR and Surrounding Areas Job Details: • Work 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s) • Visit up to 6 patients per day with 1:1 patient ratios • 30 productivity points standard required • Minimum 1 year of skilled home health experience required • First-time travelers not accepted • Weekend coverage not required • No on-call responsibilities • No floating between units required • BCLS certification required • Must have reliable personal transportation • Travel radius up to 50 miles occasionally • OASIS experience required • Any color scrubs acceptable • Charting systems: EPIC/OASIS • Free parking provided • IRS standard mileage reimbursement • Facility can call off up to one shift per pay period (every two weeks) • Must be able to float between all facilities within the state • 2 references required covering 1 year within the last 3 years • Non-billable modules average 16 hours, completed pre-start and annually Facility: This home health organization provides comprehensive skilled nursing and therapy services to patients in their homes throughout the region.
