Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 08/10/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Estimated Gross Weekly Pay: $ 2826.00 Opportunity: Home Health PT - Medford OR and Surrounding Areas Job Details: • 8-hour day shifts (4x10 and 3x12 schedules cannot be accommodated) • May visit up to 6 patients per day with 30 productivity points as standard expectation • 1:1 patient ratio • No weekend requirements • No on-call requirements • Facility may call off up to one shift per pay period (every two weeks) • Minimum 1 year of skilled home health experience required • First-time travelers not accepted • Oasis experience required • Required certification: BLS • Traveler must have reliable personal transportation • Occasional maximum travel radius of 50 miles, though system groups clients to minimize drive and windshield time • Charting system: EPIC/Oasis • Any color scrubs acceptable • Free parking provided • Mileage reimbursement at IRS standard rate • Relevant skills include acute care, outpatient clinic, rehabilitation, skilled nursing, orthopedic conditions, neurological conditions, post-stroke rehabilitation, spinal cord injury, traumatic brain injury, home exercise programs, and patient/family education Facility: This home health facility provides one-on-one patient care in a community-based setting, serving patients across Medford and the surrounding areas.
